CHANGE OF NAME 

     Guidelines for Change of Name in Indian Passport :-   

         Doccuments Required:

• Two Newspapers advertisement in original – one to be published in Indian Newspaper and another one to be published in Omani Newspaper.  (only single page of the Newspaper is required wherein the advertisement is published - having name of newspaper & date of publication. Format of newspaper advertisement is given below.   Please submit the advertisements after 30 days of its publication.  Applicant is advised that before giving the advertisement to the newspaper, the advertisement may be got attested by this Embassy, as sometimes Newspaper agencies don’t accept the advertisement for change of name unless it is attested by the Embassy.  Applicant’s presence is required with original passport and its copy at the time of attestation.  Attestation fee is RO 4/- for each advertisement.

• Sworn affidavit to be signed by the applicant in the presence of Embassy’s Official (format available at the Embassy) – Applicant’s presence is required with original passport & its copy.   Attestation fee: RO 4/-

• Passport application form duly filled in
• Photograph – two (4.5 x 3.5 cm)
• Passport in original
• Fee for Misc. Services (for change of Name in the Passport):  RO 4/-

Format of Newspaper advertisement to be put in the leading English newspaper in Oman where you are residing presently.  (Please get it attested by this embassy before giving for the newspaper advertisement in Oman)
