Documents required for Birth Registration & Passport for New Born Child

1. Application Form for Passport duly filled in , signed by both parents and child should affix his / her thumb impression in the signature boxes
    

2. Birth Registration Form
    

3. Sworn Affidavit
    

4. Passport size Photograph of Child (front view) - 2 nos.
    

5. Original Birth Certificate (after attested by the Ministry of Foreign Affairs, Sultanate of Oman) and a copy
    

6. Physical presence of Child and Parents is required OR it should be attested by one of our Honorary Consular Agents in Oman
    

7. Passport copy of Father & Mother with endorsement of spouse name on each other's passport.
    

8. Total Fee = R0 19.500 ( RO 8.800 for Birth registration ) + ( RO 10.700 Passport fee for minor child )
    

9. Total fee if applied under Tatkaal Scheme = RO 77.600 ( RO 19.500 + Tatkaal fee RO 58.100 )
