Change of name after marriage

Documents Required:  

1. Application form
2. One Passport size photograph
3. Declaration form
4. Original Passport
5. Original marriage certificate (returnable) and its photocopy (self attested). Please furnish Marriage Certificate issued by the Registrar of Marriages and photocopy of the spouse (Husband/wife’s) passport, if any

Marriage certificate issued by Church in the case of Christian and Quasi’s certificate (with a certified translation) in the case of Muslims can also be given as proof of marriage 

In case marriage certificate is not available then they can produce the joint affidavit

Change of Spouse (Wife/Husband) name due to Divorce  

Documents required:  

1. Application form
2. One Passport size photograph
3. Original Passport

4.   (a) In case of Hindu & Christian - Original Divorce Decree (returnable) and its photo copy (self attested) issued by a court in India

(b) In case of Muslim – Original Talaknama duly signed by kazi and attested by walfed board and its photocopy (self attested) 

Change of Spouse name due to Spouse (Wife / Husband ) Death

Documents required:

1. Application form
2. One Passport size photograph
3. Original Passport
4. Original death certificate (returnable) and its photocopy (self attested) issued by the Registrar of births and deaths.
