General guidelines for applying for passport services

With effect from July 25, 2011, Indian Embassy in Muscat has outsourced its Passport and Visa services through open bidding process to M/s. BLS International Services LLC-Oman. From then onwards, all the Passport and Visa applications are accepted and delivered only at the Indian Passport & Visa Service Centre located at: 4th Floor (C Block), Al Tamimah Building (Polyglot Institute) Way No 327, Building No 1340 Wattayah, Muscat.Phone : 24568970, 24568971 Fax : 24568972

The centre is open from 8am to 8pm from Saturday to Thursday. Tokens are issued from 8am to 12.30 and from 1.30 to 7pm for Passport and Visa services. The serviced documents can be collected from the service centre between 5.30 p.m to 7.30 p.m on all working days. The service charges for passport service is 450 Baisa and for Visa service is RO 1.650.The centre also provides additional services which are fully optional in nature like- filling of application, sms facility, courier facility, photocopying and photo studio facilities. It is reiterated that availing any of these facilities is not mandatory. You may also visit M/s BLS website www.blsindia-oman.com for additional information about the centre. All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Jalan bu Ali and Ibri. This is also applicable for minors and new born babies.

  1. An ordinary passport is valid for ten years from the date of issue. However, the Passport Issuing Authority (PIA) can issue passport even for a shorter duration. Minors are issued passports for 5 years or up to attaining 18 years, whichever is earlier.
  2. The Passport service sought should be in the proper prescribed application form complete in all respects- all columns are correctly and legibly filled up and no column is left blank or unfilled/un-responded or vaguely filled with a dash, tick etc. Application for the service in any other form is not acceptable. All signatures in the application form should be with ballpoint pen of black or blue ink. Those who cannot affix their signature like infants, minors or illiterates should put their thumb impression in place of signature (male should use left hand thumb impression and female should use right hand thumb impression). The duly filled in form should be accompanied with appropriate fee.
  3. A passport expired more than one year back should be supported by a Sworn Affidavit explaining the circumstances for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.
  4. Emergency service for issue of passport can be availed by payment of an additional fee of R.O. 58.100/- under the Tatkal Scheme in addition to the Normal fee (adult R.O. 28.900 (normal passport), R.O. 38.500 (jumbo passport) and minors up to 18 years of age R.O. 19.300, provided the applicant is eligible for that service.
  5. Tatkal Services has to be applied between 8am and 9am on all working days.
  6. In case the current passport is issued by other Passport Issuing Authority, Personal Particulars Form in duplicate with photos pasted should also be submitted. If the existing Passport had been issued in India then fill the Personal Particulars Form (India). If it had been issued outside India by any Indian mission, then the Personal Particulars Form (missions) should be submitted.
  7. The signature of the applicant should exactly fit in the prescribed column (4.5 c.m x 1.5 cm). The applicant should fill the surname column and the given name column correctly.
  8. In the permanent address (in India) column, please ensure that there is complete postal address including pin code written legibly.
  9. Delivery of the serviced document would be done, preferably, to the applicant himself/herself or to the person authorized by the applicant in writing and accompanied with the official receipt given to him/her at the time of submission of the application for service.
  10. The scale of fee for renewal and various passport related services and all kinds of miscellaneous services on passport are prescribed by the Government of India from time to time and are subject to change.
  11. All applications are to be accompanied with a photocopy of the prescribed pages of the passport-first two pages, last page and the page containing valid visa. A copy of ID card / residence card also to be enclosed.
  12. Photographs should be in dark colour, against white background and should be of the size of 3.5 cm x 3.5 cm. Photograph should have a frontal view, with both ears and chin to forehead being visible. Eyes should be open. There should not be any distracting shadows on the face or on the background. Head coverings are not permitted except for religious reasons, but the facial features from bottom of chin to top of forehead and both edges of the face must be clearly shown. The expression on the face should look natural.
  13. It is an offence under the Passport Act 1967 to furnish false information in the application. Passport facilities would be denied on grounds of suppression of material information, submission of incorrect particulars, willful damage/loss of passport or for unauthorized change/tampering. The Passport can be impounded or revoked for violation of one or more of the provisions of the Passport Act.
  14. It is an offence to hold more than one valid passport at a time.

(NOTE: A passport can be renewed anytime within one year before its expiry date)
 

  1. Form EAP – I (External application form) should be filled. All particulars in the application should tally with those in the current passport.
  2. Two recent passport size photos (3.5 cm x 3.5 cm) with white background, in dark dress and frontal view and without borders.
  3. Current Passport in original along with photocopies of the first, last, address page (if separate page), any other endorsement pages and visa page of the passport and additional booklets, if any. A copy of ID card / residence card also to be enclosed.
  4. For minors (up to 18 years) both parents’ passport copies are required. Both the parents have to sign the application form at the designated place.
  5. If the passport had expired more than one year back, it should be accompanied with a Delay Sworn Affidavit Form explaining the circumstances and the reasons for late renewal. The Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.
  6. In case the current passport is issued by other Passport Issuing Authority, Personal Particulars Form in duplicate with photos pasted should also be submitted. If the existing Passport had been issued in India then fill the Personal Particulars Form (India). If it had been issued outside India by any Indian mission, then the Personal Particulars Form (missions) should be submitted.
  7. In case of a new born baby, clearance of the Ministry is required if the application for the passport is made one year after the birth of the child. As such, passport cannot be issued instantly.
  8. Passport will be retained during the period of the service.
  9. All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Jalan bu Ali and Ibri. This is also applicable for minors and new born babies.
  10. All marriage certificates issued by Registrar of Marriage or any other competent authority of the concerned state should be attested by the General Administration Department (Home Department) from the state concerned where the marriage has been performed and then should be apostilled by Ministry of External Affairs, New Delhi or its branch offices in Hyderabad, Chennai, Kolkata, Guwahati and Panaji. If married abroad then the marriage certificate needs to be attested by the Indian mission in that country.
  11. If there is a change in the permanent address, then the documentary evidence in applicant’s name indicating new address like Nativity Certificate in original issued by District Collector / Tehsildar /Commissioner/ Panchayat or copies of ration card attested by the General Administrative Department of the Home department of the concerned state or the Election Commission ID card or water or electricity or telephone bill or bank statement (not more than six months old showing the address and duly attested by the concerned bank) should be submitted. In case of computer generated bank statement original bank passbook is required to be furnished along with a photocopy thereof. Aadhaar card issued by UIDAI, Government of India is also accepted as a valid address proof supported by any other additional proof.
  12. In case of correction of date/place of birth involving issue of new passport booklet, a declaratory Court Order (in original with a copy) issued by a First Class Judicial Magistrate in India instructing the Embassy to correct Date/Place of Birth duly attested by the concerned state Home Department is to be submitted.

Issue of new passport (36 pages booklet, validitiy-5 yrs) for a Minor child (below 18 years)


  1. Applicant should be ordinarily resident of Oman.
  2. If the final validity of the passport is less than one year or within one year of expiry of the passport/ on exhaustion of pages or on change in appearance.
  3. All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Jalan bu Ali and Ibri. This is also applicable for minors and new born babies,where physical presence of both the parents is required. In the event of either of the parent not physically present in Oman, he/she is required to obtain a No Objection Certificate(NOC)about issue of passport to the Minor. The said NOC should be attested by notary public and the authenticated by the Home Department of the concerned state government.
  4. Application in EAP – I form to be submitted. All particulars in the application should tally with those in the current passport.
  5. The application should be duly filled in and signed by both parents and the child should affix his / her signature or thumb impression in the signature box.
  6. Photo copy of passport of both the parents must also be enclosed.
  7. Two latest identical passport size photograph (3.5 cm x 3.5 cm without border), in plain white back ground and dress in dark colour.
  8. Current Passport in original along with photocopies of the first, last, address page (if separate page), any other endorsement pages and visa page of the passport and additional booklets, if any. A copy of ID card / residence card also to be enclosed.
  9. If the passport had expired more than one year back, it should be accompanied with a Delay Sworn Affidavit Form explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.
  10. In case the current passport is issued by other Passport Issuing Authority, in case it is issued in India then the Personal Particulars Form (India) and if it is issued outside India by any Indian mission, then the Personal Particulars Form (missions) should be submitted in duplicate with photos pasted on them.
  11. Registration of Birth & application for Passport for a New Born Child

Parents holding Indian passports should register birth of their children born in Oman with the Embassy of India, Muscat within a period of one year from the date of birth.
For registration of birth and obtaining new passport, required forms for registration of birth, issue of birth certificate and passport are to be duly filled and submitted.
If an application for passport is made after one year of the birth of the child, the case will have to be referred to Ministry of Home Affairs (MHA), Indian Citizenship Section, 26, Jaisalmer House, Mansingh Road, New Delhi-110011, in the prescribed application form for getting approval. Procedure for getting approval in such cases from MHA takes considerable time. It is, therefore, advised that parents holding Indian passports should register birth of their children born in Oman with the Embassy of India, Muscat within the prescribed period of one year without fail.

  1. Application in EAP – I form is to be duly filled in and signed by both parents. Left hand thumb impression of the child should be affixed in the signature boxes.
  2. Birth Registration Form is to be submitted.
  3. Sworn Affidavit of parents is to be submitted
  4. Two latest identical passport size photographs (3.5 cm x 3.5 cm without border), in plain white back ground and dress in dark colour.
  5. Original Birth Certificate (after attested by the Ministry of Foreign Affairs, Sultanate of Oman) along with a photo copy of the same.
  6. All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Jalan bu Ali and Ibri. This is also applicable for minors and new born babies.
  7. Passport copy of Father & Mother with spouse name endorsement should be submitted.

New Passport – in married name in case of woman

  1. Form EAP-1. All particulars other than name and signature in the application should tally with those in the present passport.
  2. Two recent photos with white background and frontal view (3.5 cm x 3.5 cm).
  3. Present passport of both husband and wife in original with a photocopy each of first, last, any other endorsement pages & page containing valid visa. A copy of ID card / residence card also to be enclosed.
  4. Original marriage certificates issued by Registrar of Marriage.
  5. Change of Name Form, in the prescribed format, signed by the applicant in the presence of the consular officer for change of name.
  6. All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Jalan bu Ali and Ibri. New Passport – (in case of woman from married name to maiden name consequent upon divorce/death of husband)

  1. Form EAP – I. All particulars in the application should tally with those in the current passport.
  2. Two recent passport size photos (3.5 cm x 3.5 cm) with white background and frontal view.
  3. Current Passport in original along with photocopies of the first, last, address page (if separate page), any other endorsement pages and visa page of the passport and additional booklets, if any. A copy of ID card / residence card also to be enclosed.
  4. In case the current passport is issued by other Passport Issuing Authority, in case it is issued in India then the Personal Particulars Form (India) and if it is issued outside India by any Indian mission, then the Personal Particulars Form (missions) should be submitted in duplicate with photos pasted on them.
  5. Divorce Decree issued and certified by the competent Court or Divorce order from Wakf Board or Death Certificate issued by the Registrar of Deaths and duly attested by the Home Department of the concerned State Government in India. If such a decree is issued in other countries, then the order should have been authenticated by the Ministry of Awqaf and attested by the Ministry of Foreign Affairs of the concerned country. The document should also have been attested by the Indian Mission in that country.
  6. Change of Name Form in the prescribed format signed by applicant in the presence of Consular Officer for change of name.
  7. If the passport had expired more than one year back, it should be accompanied with a Delay Sworn Affidavit explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.
  8.   4. In case the current passport is issued by other Passport Issuing Authority, in case it is issued in India then the Personal Particulars Form (India) and if it is issued outside India by any Indian mission, then the Personal Particulars Form (missions) should be submitted in duplicate with photos pasted on them.
  5.   9. Change of Name Form, in the prescribed format, signed by applicant in the presence of Consular Officer for change of name
  10. New Passport – (all other cases other than woman seeking change from maiden name to married name or from married name to maiden name consequent upon divorce/death of husband or applying for change of her name/spouse name in her passport in the case of re-marriage)

  2. Two recent photos with white background and frontal view (35mm x 35 mm).
  3. Current Passport in original with photocopy (of first, last, address page if separate page, any other endorsement pages and visa page with additional booklets if any.A copy of ID card / residence card also to be enclosed.
  4. Change of Name Form, in the prescribed format, signed by applicant in the presence of Consular Officer for change of name.
  5. Paper cuttings, in original, of two leading daily newspapers, notifying the change of name. One, preferably English daily newspaper, should be of the area of applicant’s permanent address in India and another daily newspaper of the area of applicant’s present address. *Format of newspaper notification is given below: I, ____________son/daughter of (Names of both parents)_______________, holder of Indian Passport No__________, issued at _____________on _______________, permanent resident of ________ (full postal address in India to be given) and presently residing at _____(full postal address in Oman to be given) do here by change by name from __________________ to _________________, with immediate effect.
  6. If the validity of the present passport expired more than one year back, Delay Sworn Affidavit for late submission, signed by applicant in the presence of Consular Officer
  7. In case the current passport is issued by other Passport Issuing Authority, Personal Particulars Form in duplicate with photos pasted should also be submitted. If the existing Passport had been issued in India then fill the Personal Particulars Form (India). If it had been issued outside India by any Indian mission, then the Personal Particulars Form (missions) should be submitted.
  8. All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Jalan bu Ali and Ibri. New Passport in lieu of lost Passport


Requirements:

  1. Form EAP – I. All particulars in the application should tally with those in the current passport.
  2. Two recent photos with white background and frontal view (3.5 cm x 3.5 cm).
  3. Photocopy of the lost passport. A copy of ID card / residence card also to be enclosed.
  4. A copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation as well as a letter issued by the local immigration subsequent to the police report along with photocopies of the FIR and the Immigration letter.
  5. A letter addressed to the Embassy, from the sponsor/employer (from parent in case of a minor) of the applicant on the company’s letterhead intimating about the loss of the applicant’s passport and explaining circumstances leading to the loss of the passport. One photo of the applicant should also be pasted on the letter. Following is the format of the letter from the sponsor/employer: “This is to inform you that Mr./Ms. ______________ s/o, d/o, w/o _____________ holder of Indian passport No._____________ issued at ________________ on ______________ whose photograph is pasted and stamped( company visa)/signed below(, is working under our sponsorship. He/she has genuinely lost his/her above-mentioned passport. The passport was lost on ________________ in the following circumstances _____ (Please explain in detail). Kindly issue a new passport to him/her.

  6. A copy of the passport or any other identity proof of the sponsor.
  7. Original with a copy of one or more of the photo-affixed identity cards to establish identity and nationality. The original will be returned after verification. The photo identity card could be (a) Identity Card (b) Labour Card (c) Health Card (d) Driving License and (e) Election commission card.
  8. Newspaper advertisement, in original, in two leading local dailies (one each in English and Arabic) as per the following format (Please enclose full sheets of the newspaper and not cuttings thereof): “Indian Passport No.___________ dated _________ issued at ____________ and held by Mr./Ms. _____________ s/o, d/o, w/o ____________________ has been genuinely lost. The passport was lost on ____________ in the following circumstances. Finder please contact __________________,”
  9. An affidavit explaining
    • The circumstances under which the passport was lost
    • Details of travel on the lost passport
    • Details of travel concessions including Transfer of Residence benefits, Foreign Exchange availed of, imports made by the applicant on the lost passport;
    • Places of stay and work in Oman since arrival.
    • The format of the affidavit is as follows-

New Passport in lieu of Damaged Passport

Requirements:

 

  1. Form EAP-I. All particulars including name and signature in the application should tally with those in the damaged passport
  2. Damaged passport, in original, along with a photocopy. A copy of ID card / residence card also to be enclosed.
  3. Two recent photos with white background and frontal view (3.5 cm x 3.5 cm).
  4. Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation. (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage)
  5. A letter addressed to the Embassy, from the sponsor/employer (from parent in case of a minor) of the applicant on the company’s letterhead explaining circumstances leading to damage of the passport. One photo of the applicant should also be pasted on the letter. The format of the letter suggested is as follows: “This is to inform you that Mr./Ms. ______________ s/o, d/o, w/o _____________ holder of Indian passport No._____________ issued at ________________ on ______________ whose photograph is pasted and stamped( company visa)/signed below(, is working under our sponsorship. He/she has genuinely damaged his/her above-mentioned passport. The passport was damaged on ________________ in the following circumstances _____ (Please explain in detail). Kindly issue a new passport to him/her. Signature of the sponsor along with a photocopy of the passport
  6. Original with a copy of one or more of the following photo-affixed identity cards to establish identity and nationality. The original will be returned after verification. The photo identity card could be (a) Identity Card (b) Labour Card (c)Health Card (d) Driving License and (e) Election commission card.
  7. Newspaper advertisement, in original, in two leading local dailies (one each in English and Arabic) as per the following format. Please attach full sheets of the newspaper and not cuttings thereof (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage).The format of the advertisement suggested is as follows: “Indian Passport No.___________ dated _________ issued at ____________ and held by Mr./Ms. _____________ s/o, d/o, w/o ____________________ has been genuinely damaged. The passport was damaged on ____________ in the following circumstances. Finder please contact __________________,”
  8. An affidavit signed by the applicant explaining 
    a) The circumstances leading to the damage of the passport;
    b) Details of travel on the damaged passport;
    c) Details of travel concessions including Transfer of Residence benefits, Foreign Exchange availed of, imports made by the applicant on the damaged passport;
  9. In case the current passport is issued by other Passport Issuing Authority, in duplicate with photos pasted should also be submitted. f the existing Passport had been issued in India then fill the Personal Particulars Form (India).
